The health program has been designed to improve, protect and promote the health of the children. However, the success of the program depends on the cooperation of the parents. To facilitate your cooperation and ensure its success, we have outlined briefly for you the important aspects of the health program.
According to the directives of state, county, township, and school board policy we are mandated to make certain requests of you before your child can attend class. We have outlined these requirements for you and anticipate your full cooperation in these matters.
IMMUNIZATION REQUIREMENTS
D.P.T. (Diphtheria, Pertussis and Tetanus), at least four doses of DPT provided one dose is given on or after the fourth birthday. As of September 2008, 6th grade students will be required to show an additional dose of Tdap.
Polio – at least three doses provided one dose is given on or after the fourth birthday.
Measles –two doses after age one
Rubella –at least one dose after age one
Mumps –at least one dose after age one
Hepatitis B – series of 3 doses.
Varicella – at least one dose after age one or verification of disease by doctor, serologic evidence or parent.
Meningococcal – for 6th grade as of September 2008
IMMUNIZATION RECORD MUST BE SUBMITTED TO HEALTH OFFICE ON OR BEFORE THE FIRST DAY OF SCHOOL.
PHYSICAL EXAMINATIONS
Physical examinations are recommended annually and are required for all new students entering the Guttenberg School District. The health record form to be completed by your physician is included under the link forms.
TB-TESTING
If your child is coming from a TB designated testing area, a Mantoux test must be administered. You can verify if you need a Mantoux test by checking with your pediatrician or our school nurse.
EMERGENCY AND HEALTH ACCIDENT FORM CARD
These cards are distributed yearly to all students. These cards must be completed in full and returned to the school for use if a child is injured or becomes ill at school. Please take the time to carefully fill out and complete both sides with signature.
YEARLY HEALTH SCREENINGS
Vision, hearing, and scoliosis screenings are conducted according to NJ State Standards of Practice. Heights, weights and blood pressure are done annually to determine the growth pattern of the children.
ILLNESS AND INJURIES AT SCHOOL
Students who are ill or injured at school are seen in the health office. Parents will be contacted if their children need further care.
ILLNESSES AND INJURIES AT HOME
Please call the school in the morning if your child is going to be absent. All illnesses and injuries should be reported to the school. Children with a fever, productive cough, cold, rash, or inflamed eyes should be kept at home. Students returning to school after being absent must have a note from a parent or doctor.
COMMUNICABLE DISEASES
Communicable diseases should be reported to the school nurse immediately upon diagnosis. Quarantine regulations to be followed are issued by the state and local Board of Health.
MEDICATION
Any medication that is to be given in school, whether over the counter or prescription, requires the authorization of a physician and the parent. Medication forms can be found on the district website under health forms and must be renewed every year.
